The lastest (2 April 2020) Coronavirus PCN update regarding high-risk patients.

High Risk patients

Patients who have been identified from their medical records as being particularly high risk will be receiving text messages and letters from the NHS to start 'shielding' themselves for at least 12 weeks after the text or letter is received.

If you have had a letter and do not have family, friends or community support to access food and medicines you can register for help on the government dedicated website.

Rutland Questions

In Rutland, the RISE team, a joint team between the 4 GP practices and Rutland County Council, will be telephoning shielded patients to find out whether they need help or support to follow the shielding recommendations for the next 12 weeks.

The RISE ream will be asking the 6 "Rutland Questions"

A. Who is helping you? What happens if that person becomes unwell and cannot assist?

B. How are you getting food?

C. How are you getting medicines?

D. Do you have an urgent question about your medical condition?

E. Do you have other needs?

F. Do you or the person helping you have on-line access?

If you do not have a care plan in place, your GP or one of Rutland Health's Admiral Nurses may contact you to discuss your care preferences and to offer you the chance to complete a ReSPECT form which records your preferences for clinical care in emergency situations where you are not able to make decisions or express your wishes. 

You can also consider

  1. Enhanced Summary Care Record - consenting to sharing of your records with hospitals and other healthcare organisations involved in your care.
  2. Repeat dispensing - allowing your registered pharmacy to re-order your medication for you for the next 4 months.

The list of high risk patients is being updated. If you have not received a letter yet but feel that you in the high risk category, please follow the shielding precautions until the NHS sends you a letter. 

  1. Strictly avoid contact with someone who is displaying symptoms of coronavirus (COVID-19). These symptoms include high temperature and/or new and continuous cough.
  2. Do not leave your house
  3. Do not attend any gatherings. This includes gatherings of friends and families in private spaces, for example, family homes, weddings and religious services.
  4. Do not go out for shopping, leisure or travel and, when arranging food or medication deliveries, these should be left at the door to minimise contact.
  5. Keep in touch using remote technology such as phone, internet and social media.

Do use telephone or online services to contact your GP or other essential services.

All Rutland practices can now offer telephone and video consultations and you can access the practices for non-urgent queries by email.
