    The issues highlighted included delayed transfer of care, discharge from hospital without sufficient support in place, discharge from hospital when the individual requires rehabilitation rather than in-home support and confusion upon returning home.

    We heard from the public that some confusion existed surrounding the provision of support to military veterans in the county so we held a workshop to explore the issues.
