Healthwatch Rutland, in partnership with the Better Care Fund, are running a project until March 2017 to gather the experiences of people who have been in hospital and then either gone into residential care (temporarily or permanently) or have gone home with social care support in place.
If you have been treated in hospital but needed continued support afterwards (either in residential care or at home), Healthwatch Rutland wants to hear about your experiences. What was done well, and what could be done better. Your information will be used to inform health and social care providers to make services better for the people of Rutland.
Your information will be anonymous and you only need to tell us what you want to. Your experiences can be sent to us (by post or email), we can take your information on the phone, or a Healthwatch volunteer can visit you (in hospital, care home or your own home).
